Tips on learning vocabulary for speaking and listening?

I've been studying actively for a few years now, but my vocabulary is still very limited. I can't have any conversation with Chinese co-workers without saying 听不懂 a dozen times. Did any of you have this problem at some point? If so, what did you do to get to the point where you can speak to natives with basic fluency?

My current method is using the Spoonfed Chinese anki deck to learn new words. Front of the cards are only Hanzi. I first speak the sentence aloud and then check if I was correct with the pinyin on the back of the card. Maybe this is bottlenecking the rate at which I assimilate new words?
